Description:
Form 4.1 contains a notice meeting of members to be used to call a meeting of the members of a manager managed LLC. The form can also be used to call a meeting of the managers or to call a meeting of the members of a member managed LLC, although in both these cases, a less formal notice will often be used.
The form also contains a certificate of mailing and a waiver of notice. In addition, the form contains a proxy form with which an LLC member can authorize someone else to vote his or her interest.
